About the NPMA-33 form. The NPMA-33 is copyrighted by the National Pest Management Association and may only be produced by licensed printers and authorized digital providers. PestRouteDesk is not currently an authorized NPMA-33 provider. Our plan is to apply for authorization before launch; until then we will not produce the NPMA-33 itself. Several states require their own WDI/WDO form instead (NPMA lists Alabama, Arizona, California, Florida, Georgia, Hawaii, Louisiana, Maryland, Mississippi, Nevada, North Carolina, Oklahoma, South Carolina and Texas).

Can I use this for NPMA-33 reports on day one?
Not unless we have been authorized by NPMA by then. We will state our authorization status on this page and on the product status page.
Does a WDI report include wood decay fungi?
The NPMA-33 covers insects only. NPMA publishes a separate, non-copyrighted model Wood Destroying Organism attachment for areas that require fungus reporting.
